Delta States Rural Development Network Grant Program

This announcement solicits applications for the Delta States Rural Development Network Grant Program. The purpose of the Delta States Rural Development Network Grant Program (Delta) is to fund organizations located in the eight Delta States to address unmet local health care needs and prevalent health disparities in rural Delta communities. In practice, the Delta Program provides resources to help rural communities develop partnerships to jointly address health problems that could not be solved by single entities working alone. Applicants may also propose to address a second priority area as demonstrated by a needs assessment. Applicants may propose funding for up to 3-years starting from August 1st, 2013 to July 31st, 2016. Award amounts are $25,000 per county served and are based on a previously defined service area. Further information regarding the funding amounts are in the announcement. ORHP expects to fund 12 grantees.

Health Center Program New Access Point Grant Application Technical Assistance

The purpose of the Health Center Program New Access Point (NAP) funding opportunity is to improve the health of the Nation’s underserved communities and vulnerable populations by increasing access to comprehensive, culturally competent, quality primary health care services. The Health Center Program New Access Point funding opportunity is an important element of HRSA’s commitment to improving and expanding access to needed health care services. Subject to the availability of appropriated funds, HRSA anticipates awarding approximately $19 million to support an estimated 25 New Access Points grant awards in Fiscal Year 2013, supported by the Affordable Care Act (P.L. 111-148). The maximum annual amount of Health Center Program funding that can be requested in a NAP application is $650,000.
Eligible Applicants
Organizations eligible to compete for NAP funds include public or nonprofit private entities, including tribal, faith-based, and community-based organizations. Applications may be submitted from new organizations or from existing grantees to expand their network of service delivery sites to serve new underserved populations.
Under the Fiscal Year 2013 Health Center Program New Access Point funding opportunity, applicants must propose to establish a new access point that:
- Provides comprehensive primary medical care as its primary purpose;
- Provides services, either directly onsite or through established arrangements, without regard to ability to pay;
- Ensures access to services for all individuals in the service area/population; and
- Provides services at one or more permanent service delivery sites.

Small Healthcare Provider Quality Program

The Office of Rural Health Policy (ORHP) is pleased to announce the release of the FY 2013 Small Health Care Provider Quality Improvement (Rural Quality) Grant Program (Announcement number: HRSA-13-159). The purpose of the Rural Quality Program is to provide support to rural primary care providers for implementation of quality improvement activities. The ultimate goal of the program is to promote the development of an evidence-based culture and delivery of coordinated care in the primary care setting. Additional objectives of the program include: improved health outcomes for patients; enhanced chronic disease management; and better engagement of patients and their caregivers. Organizations participating in the program are required to use health information technology (HIT) to collect and report data. HIT may include an electronic patient registry (EPR) or an electronic health record (EHR), and is a critical component for improving quality and patient outcomes. It is expected that organizations will have implemented an HIT system by the time of award. Sustainability is also beginning to be an emphasis of this program. It is a priority of the ORHP for grantees to sustain improvements, integrate quality improvement into their organizations, and spread quality improvement efforts to other conditions and populations. Applicants may propose funding for up to three (3) years from September 1, 2013 to August 31, 2016. The maximum award is up to $150,000 per year. We expect to fund approximately 40 grantees.
